Office Policies
Please contact our office at (410) 284-1133 to schedule an appointment. We see patients by appointment only. Insurance We accept most medical insurance. Please contact our office to verify. Please bring your child's medical insurance card with you to each office visit. Check in Please arrive 15 minutes prior to your appointment to complete paperwork. Payments and fees All co-pays are due at the time of your appointment. Your child will not be seen without receipt of the co-pay. New Patients If you are new to our practice, please bring all pertinent medical records or have your child's records transferred to us before your first appointment. You can also download and complete our registration forms before you arrive. New Patient Forms Financial Policy Acknowledgment Referrals We require 48 hours notice to process referrals. Form fees There is no fee for completing school, sports, camp and other forms during your office visit. However, there is a $2.00 fee for completing forms without an office visit. Please have all necessary forms with you at the time of your visit to avoid a fee. Medical Records If you request a copy and transfer of your child's medical record, you must sign a "Release of Medical Record" form. There will be a per page copying fee for this service. Accessibility Our office is handicapped accessible with a lift for wheelchairs or patients unable to walk steps. Location Our office is located at 2112 Dundalk Avenue, Baltimore, Maryland 21222 (Lower Level of Johns Hopkins at Greater Dundalk). Office Hours Monday-Thursday: 9 a.m. - 7:30 p.m. Friday: 9 a.m. - 5 p.m. Saturday: 9 a.m. - noon* * We may close earlier on Saturday if there are no patients to see, so call early. Need Help?
